Our Basic Connected Care Program provides you with portal access to your physician within 5-7 business days.

Subscribers at this level will also receive a 30 minute annual physical exam and evaluation. This visit can be used to address complex medical conditions, or in depth healthy lifestyle changes, including nutrition, physical activity, stress, and sleep.  This enables you to unlock answers and solutions that might have previously eluded you in a typical 15 minute clinic visit.

Because we accept insurance for all covered services there is no limit to the number or frequency of medically necessary visits.  Follow up appointments are usually scheduled as 30 or 60 minutes, your choice.

Basic subscription patients have access to limited same day appointments, as well as our state of the art online patient portal where they can easily access their medical records, request prescription refills, schedule appointments, and more.
